AND
FURNISHING
OF
SERVICE
PPLICATION
Application

contracts

the

All
the
regula-
tions
contained
in
the
tnrtfr
shall
be
part
of
such
contracts
for
service
and
nll
facilities
and
service
rendered
thereunder
are
subj
ct
to
the
Company's
tarlf!, rules
and
regnl tlons a·s on file
at
the
Com-
pany's
business
offic
.

Requests from
subscribers
for
additional
service
or
equipment
shall
be
made
in
wrft-
Jng. Upon
approval
or
installation
thereof,
such
service
or
eQuipment Is deemed
to
be covered by
the
original
contract,
except
that
such
item
of
additional
service or
equipment
so
installed
is
subject
to
the
approPriate
minimum
contract
term
in
accordance
w1th
the
Company's
regulations
governing
initial
con~
tract
periods.

to
the

charges.
AnY
change
in
the Company's
rates
shall
effect
the
alteration
of
all
contracts
so
as
to
render
such
contracts
in
con-
formity
therewith.

such
alteration
shall
take
effect
without
notice.
The
subscriber
shall
not
assign
his
contract
with
the
Company
nor
any
rights
thereunder.
ADVANCE
PAYMENTS
At
the
time
of
application
for
service.
the
aPPHcant will be
required
to
pay
the
proper
service
connection charges.
together
with
an
advance
payment
equal to one
month's
charges
for
exchange
service,
equipment, mlleage, listings, etc.
Notwithstanding
the
preceding
paragraph,
and
in
addition
to
the
advance
payment
therein
provided,
an
applicant
or
subscriber. whose
credit
standing
is
not
established
to
the
Company's
satisfaction,
may
be
required
to
make
a
further
advance
payment
of
an
amount
which
the
Company
considers
sufficient
to
cover
not
more
than
six
months'
charges
for
exchange
service, toll service,
equipment.
mileage, listings,
and
all
other
services
and
charges
for
which
the
Company
is
entitled
to
receive
payment.
Charges
for
service
rendered
are
applied
against
such
advance
payment.
If.
in
the
opinion
of
the
Company,
the
subscriber's
net
credit
balance
is
at
any
time
Insufficient
to
cover
the
estimated
serYice
charges
referred
to,
the
CompanY
may
require
a
further
advance
payment
or
suspend
a.nd
terminate
service
in
accordance
with
the
regula-
tiOns
contained
in
the
tariff.
INSTALLATION
OF
WIRING
~xcept
where
otherwise
provided
in
the
Company's
tanffs
or
by special
agreement,
all
work
of
installation
of wiring
and
equipment
on
the
appllcant's
premises
is done by
the
Company,
and
the
Company
attaches
~ts
instruments,
equipment
or
lines
only
to
wiring
mstalled
by
its
own employees.
TELEPHONE
NUMBERS

The
subscriber
shall
have
no
property
right
in
the
telephone nu.mber assigned
to
him
or
any
right
to
c~tinuance
of
service
through
any
particular
central

ce.
and
the
Company
may
change

anything
contained

Regulations,

Compan,.

establishment

phone
service
with
a
subscriber
for
any
eharces
that
have
accrued.

demand
payment
wlthln
a
period
of
not
less
than
three
days,
and

pliance
with
such
demand
for
payment,
the

relating
to

of

Regulations••
shall
apply.
All
charges
Incurred
by
the
subscriber
are
payable
at
the
Company's
business

at
any
agencies
duly
authorized
to
receive
such
payments.
Failure
to
receive a
bTil
showing
the
amount
owed
bY
the
subscriber
to
the
Company
shall
not
relieve
the
subscriber
from
the
responslbtltty
of
maktnc
prompt
payment
of
such
amount
to
the
Company.
The
subscriber
shall
be Uable
to
the
Company for
all
tolls
and
charges
for
all
calls
originating
on
his
telephone
regardless
of
who
may
originate
such
calls.

also
for
all
calls
received
at
his
telephone
the
charges
for
which
are
accepted
by
any
person
recelv~
ing
such
calls
regardless
of
who
may
accept
such
charges.
The
Company,
however, reserves
the
right
to
collect
such
charges
from
the
person
placing
the
call
or
who
may
otherwise
be responsible
for
the
charges
incurred.
Messengers
may
be used,
If
available,
at
the
request
of
the
person
placing
a toll call.

trying
to
locate
the
called
person
where
he
cannot
be
reached
bY
telephone,
provided
the
call1ng
person
agrees
to
pay
the
cost
of
such
service.
If
objection
tn
writing

not
received by
the
Com-
pany
within
thirty
days
after
a
statement
of
account
is
rendered.

statement
shall
be deemed
to
be
correct
and
binding
upon
the
subscriber.
MAINTENANCE
AND
REPAIRS
All
ordinary
e:xpense
of
maintenance
and
repairs
unless
otherwise
specified
in
the
subscriber's
applica-
tion
or

the
Company's
tariffs.

Upon
termination
of
service
any
equip-
ment
furnished
by
the
CompanY
shall
be
required
to
be
in
good
condition.

the
case
of
damage
to
or
destruction
of
any
such
equipment,
not

to
ordinary
wear
and
tear,
the
subscriber,
on
demand,
shall
pay
to
the
Company
the
cost
of
replacing
the
equipment

restoring
it
to
its
original
condition.
The
subscriber
shall
not
rearrange,
disconnect
or
remove,
or
permit
others
to
rearrange,
disconnect
or
remove,
or
other-
wise
interfere
with
any
apparatus
or
wiring
installed
by
the
Company,
except
with
the
written
consent
of
the
Company.
The
Company
reserves
the
right
to
enter
Into
agreements
with
railroad
companies
or
other
parties
whereby
they
may
be
required
to
install
and
maintain
telephones
and
telephone
apparatus
furnished
by
the
Company,
when
such
telephones
and
apparatus
are
located
on
the
;ailroad
premises
or
in
other
places
involving
unusual
hazards,
or
when
St

convenient
to
both
parties
